Violation of Child Custody Orders

In cases involving violations of child custody orders, please complete this form.  Your options with the completed form are as follows:

1. If you have your Order to Show Cause and your family law case number, you may take all documents directly to the Family Court at 1500 Court Street, Room 319 and request the form be added to your file. 
2. If you do not have your Order to Show Cause, you may go to the Family Law Facilitator located in Room 115, 1st floor, of the Shasta County Courthouse to complete any other necessary documentation.  
   
3. You may take the completed paperwork to your next court date.

For violation of restraining orders, complete the Violation of Court Order Form. The completed form and supporting documentation can be dropped of or mailed to:
